Please fill out this form to request a performance. Please note there is a performance fee associated with each performance which does not include AV (microphones, speakers, stage monitors, etc.). This fee is set by the artists, not United Way. Preferred method of payment is a cheque on or before the date of your event made out to PlanIt Sound.
A United Way or PlanIt Sound representative will also attend the event.
